There are two ways to get the fluoroscopy license renewal form.

Option 1: You normally receive your fluoroscopy license renewal form about 3 months before your license expires. The form arrives to the address that you registered with the RHB the last time you renewed.

Option 2: Email [e-mail] and our team will reply with an electronic version of the form.
